definitely BLACK ink. And Print Legibly/neatly, IF you cannot type the info.instead.

If I sent it off with Blue ink, will it cause an issue? Do I need to redo it?

Black ink. First name-Middle name:Last name, Beneficiary (or BENE)

I asked on two different Telegram groups and got three answers - all saying to autograph/endorse in blue ink
